MEMBERSHIP DESCRIPTION AND PAYMENT SCHEDULE

 

Minimum Term. All Memberships are for a minimum period of twelve (6)months.

 

· Payment Schedule. The first payment occurs the first day at each month.

GENERAL TERMS AND DISCLOSURES

 

· Cancellation: If you want cancel your skin club program you have to notice Bastos Beauty one month before your next month. If you cancel your membership before six month you will not get your birth facial free. to qualify for the free facial in the month of your birthday you have to complete 6 months of membership. 

 

.Appointment rescheduled: 8 hours before your appointment no fee, after 8 hours a fee $10 will be charge at your card. 

 

· Transferability: Memberships are granted on an individual basis, are non-transferable, and are not applicable to others.  

 

I agree that Bastos Beauty will charge $ 70 every first day of each month, for 6 months. Starting on the date of submission of this form.
